Supervisor Instructions on Personalized OMR forms
1.0 Candidates should handle the personalized OMR forms with care and not write or put marks on any other part of the OMR form apart from shading the correct answers.
2.0 The OMR forms come already pre-printed with the index number, name of the candidate and the centre name and they are subject specific. The candidates are therefore expected to verify that the forms that they have been issued with have their correct index number, name and centre name and that they are for the correct subject.
3.0 The OMR form has a provision for shading absent candidates at the top right hand side. This part is clearly indicated for official use only and the Invigilator(s) should shade this part for candidates who are absent. Candidates should not shade this part.
4.0 The supervisor must ensure that the candidates are properly guided on how to shade the answers on the OMR form. This should be done during the rehearsal day.
In cases where a candidate misses a personalized OMR form, then the candidate will answer on the question paper then submit for manual marking.
